Transition from Schools

What is Transition?

Transition is a programme designed to help students make a smooth transition from school to work, further education or a community programme.

The Transition Programme is specifically for students who face challenges as a result of their disability.

We work collaboratively with students, schools, parents and employers to plan and provide employment options and/or further education. 

We do this by providing:

  • Opportunities for supported work experience.
  • Vocational planning.
  • Community participation.
  • Family/whanau/school/provider collaboration.
  • A work experience profile.
  • Individualised ongoing support.
